Public Health Nursing Interventions Public Health Nursing (PHN) aims to improve population well-being by promoting health and preventing disease among all people in communities (Public Health Nursing, 2013). PHN uses primary, secondary and tertiary prevention to help improve the health of communities. The PHN process is applied at all levels of practice. Interventions are “actions taken on behalf of individuals, families, systems, and communities to improve or protect health” (Stanhope & Lancaster, 2012, p. 191). This document will identify PHN interventions such as screening, awareness, referral and follow-up, health teaching, and counseling that were identified in the PHN in the 21st Century project that this author completed as part of the PHN experience. This document will also identify whether interventions were at the community, system or individual/family level. Community level The focus of practice at the community level is to change the norm, attitudes, practice behavior and increase the knowledge of the entire community regarding the importance of immunization. and the consequences of failure to immunize (Stanhope & Lancaster, 2012). The primary goal of PHN is to prevent illness or disease from occurring. Vaccine administration is a primary intervention provided by the PHN. Local public health nurses provided information packets to public schools, senior centers, daycare centers and other local businesses about the need for flu shots and where to go for free or affordable flu shots. In the package was a contact card that the community could return to PHNs. This was a way for PHNs to track the number of people who responded to the distributed message. The nursing process is applied every day at all levels of nursing practice. Public health nurses' interventions are targeted to the entire population and guided by an assessment of community health (Public Health Nursing, 2013).
